Formal Investigations are usually associated with more significant incidents with multiple causes. It is an in-depth analysis of the incident, with the purpose of ultimately identifying the root causes, thereby enabling appropriate corrective action to be taken to minimise the risk of recurrence and improve health, safety and environmental management, following the incident. This Standard outlines how to commission and conduct a Formal Investigation, including the appointment of Health and Safety Representatives to the investigation team; as well as the communication of the findings and monitoring of any actions arising from the investigation.

Purpose

The purpose of this Standard is to define LU's requirements for the Formal Investigation of incidents.
