Reference (a) is the basic directive for OPREP-3 reporting of events and special incidents that may attract national or high-level U.S. Navy interest. Per reference (a), NETC will be an action addressee on OPREP-3 reports. Reference (b) is the basic directive for reporting aviation mishaps and hazards. In addition to the requirements of reference (b), the following reports are required by NETC:
a. NETC will be an action addressee on the following message reports:
(1) Initial mishap reports (OPNAV Report Symbol 3750-20) and mishap investigation reports for all Class A and B mishaps involving NETC claimancy resources.
(2) All aviation hazard reports (OPNAV Report Symbol 3750-19) involving NETC claimancy resources.
b. Chief of Naval Air Training will ensure that a telephone report of a unit experiencing a Class Alpha or Bravo aircraft mishap is made to NETC Headquarters within 1 hour of occurrence. The phone number for the NETC Quarterdeck is (850) 452-4000/DSN 922-4000. After normal business hours, this number will automatically forward to the NETC Staff Duty Officer. The NETC Staff Duty Officer can always be reached at (850) 554-5312 (NEXTEL) and via Blackberry e-mail at sdo.netc@navy.mil.
c. In addition, a telephone report should be made to NETC Headquarters regardless of mishap class whenever press interest might ensue or a mishap of an unusual nature occurs (e.g., a gear-up landing, things falling off aircraft, or civil aircraft mishap involving Navy property or personnel).
Purpose.
To establish supplemental requirements for the distribution of certain reports of aviation mishaps and hazards within the NETC claimancy.
