The purpose of this document is to develop and present a shared understanding of the structure and content of data needed to support healthcare processes. This conceptual model forms the foundation for more detailed data representations in clinical information and the structure and content of data presented in the various types of electronic health and patient records. This document provides descriptions and graphic depictions of those activities and data structures specific to clinical healthcare and population health services.
