Description of Driving Event:

While an employee was climbing out onto a steel beam to inspect debris nets on the 16th level of the VAB, a beam clamp attached to the employee's belt struck a beam and fell 16 floors to the transfer isle of the VAB. The clamp fell near personnel in the transfer isle. The clamp was not properly tethered. Continuing emphasis is required to ensure that personnel are knowledgeable of and comply with tethering requirements.
