Lessons Learned – Forklift load test

Following the repair of a forklift, a 100% load test was being performed with 4,995 pounds of weight.
The weight configuration consisted of a large metal weight of 4,745 pounds and several smaller weights on a metal pallet. The load test only requires lifting the weight approximately 12 inches and tilting it up and then down. When the weights were tilted down the 4,745 pound weight slid off the pallet onto the floor and tipped over. There was no injury to personnel and minor damage to the floor.
