Description of Driving Event:
Due to resource constraints and a past history of success with a specific commercial vendor the Project decided to reduce the amount of in-house Quality Assurance (QA) activities. Thus, there was a reduction of QA with regard to items the Project owed the vendor (design drawings) and deliverables the vendor owed the Project (hardware). Unfortunately, this reduction of in-house QA combined with the unknowns associated with one-of-a-kind or cutting edge technology lead to problems being discovered late and/or items having to be accepted with waivers for nonconformance.
