Description of Driving Event:

Since the award of the Basic Space Station Contract numerous changes have been authorized, but there was no consistent, established method of authorizing and notifying the other Space Station locations of their respective changes. The result was that each sites' assigned work, when added together, did not equal the total work required by the program at any particular point in time.

Root cause: a process was never defined (or effectively implemented) to reconcile the work of the decentralized work sites with the entire program.
