Description of Driving Event:
The proof load test operator installed a 3 leg sling H77-0235 in the tensile machine to proof load test a "D" and/or an "O" ring to 72,000 lbs and 52,000 lbs. The tensile puller pump motor stopped and the operator shut down the machine immediately and discovered tensile puller over pulled the "D" ring, deforming it, also distorting 3 pins in the tensile puller. The "D" ring was distorted beyond a salvageable limit.
