Description of Driving Event:

Technicians were performing radiographic inspection of the insulation area in the atlas stage thrust section utilizing a 100KV machine. The tubehead was placed inside the thrust area taped into position and exposures were made to the outside. The tape failed to hold the tubehead and it slipped down 6-8 inches hitting a threaded area on a strut. There was only visible damage to the top of 2-3 threads.
