General
This leaflet is intended to provide prompts for use by staff when investigating and reporting incidents on the railway, such as:
• irregular failures.
• a person under a train
• an incident at the platform/train interface
• an incident involving a lift, escalator or moving walkway
• asset failure related delays
• allegation that the train doors has caused injury.
This leaflet should be used to help with investigations and to complete Electronic Incident reporting Forms (EIRFs).
In all cases, when undertaking or reviewing an incident, the following must be considered and appropriate records taken (photos can be very helpful).
• witness statements - staff and public or customers and contact details
• cleanliness and dryness of the area
• weather conditions (such as lighting, rain, etc...)
• voice, video or photographic evidence available
• preservation of evidence - collection, recording and storage
• drugs and alcohol policy
• attendance of emergency services
• should include a detailed description of the "incident"
• fault reference number
• who attended/was involved
• location - details, points of reference
• previously reported defect
