The FAIR process is used when the investigation of a safety incident identifies that actions or behaviours of Train Operators or Service Control staff were among the causes. The manager who is accountable for deciding the next steps, including any actions affecting the people involved, applies the flowchart to the investigation findings and records the recommended outcome in the fields at the end of this guide.

For Train Operators, safety incidents are defined in the Safety Related Staff Error Incident Management Guidelines.

Purpose

This guidance explains the Flowchart Analysis of Investigation Results - (FAIR) - process and how the accompanying FAIR flowchart is applied by managers in Line Operations.
