This procedure applies to OCCs requiring a Level 1 investigation where an Operations function was affected.

 

NOTE: Typical examples are points run throughs, SPADs, level crossing incidents, safety critical communication failures, failure to apply the correct rules and regulations.

  

PURPOSE

This procedure is to provide the briefing of information, where an Operational Close Call (OCC) has occurred the relevant information is disseminated to all frontline operational staff. To mitigate operational risk by allowing frontline operational staff to learn from other operational mistakes made on the network.
