Introduction
Due to an AWS electro-inductor failure, the DR relays of two signals were disconnected. They remained disconnected for several months, and drivers began to ignore the double yellows resulting from the disconnections. Eventually, this led to a SPAD. It was subsequently discovered that a further long term DR disconnection had been made in the neighbouring area - with serious safety and performance implications.
