In the Constellation Program, safety risks were captured using a risk management tool. This led to duplicated efforts and misclassification of the risks. To prevent this in future programs, assessments of safety risks should be done in the hazard analysis effort. The safety ranking, safety tab, and safety assessment functions should be removed from risk management tools used in future programs.
