This Standard applies to situations where:

  • Normal verbal or written reporting methods have failed or a written acknowledgement is required
  • The hazard or condition is not a maintenance item and therefore cannot be reported in the normal way
  • The hazard being reported is outside the member of staff's normal work area and they are unsure of who to report it to.

Purpose

The purpose of this Standard is to define LU's alternative reporting arrangements for health, safety and environmental matters.
