This Standard applies to all LU Directorates and covers the notification, recording and reporting of incidents, dangerous occurrences and near misses, both within LU and to external parties.
Details of incidents requiring a Formal Investigation and the arrangements for conducting a formal investigation are contained in the LU Category 1 standard S1558.
Purpose
The purpose of this Standard is to define the requirements to ensure that a robust method of recording and investigating incidents is maintained to prevent recurrence.
