PURPOSE
This directive establishes the Goddard Space Flight Center (GSFC) procedures for investigating mishaps and close calls, including the establishment of investigation boards and the development, implementation, and evaluation of corrective actions and lessons learned. This directive is the GSFC implementation of the investigational aspects of NPR 8621.1.
APPLICABILITY
This directive applies to Type B mishaps, Type C or Type D mishaps, and close calls, and to Type A mishaps that have been delegated by NASA Headquarters to GSFC for investigation. (See the table in Appendix A, Organizational and Management Notification, GPR 8621.2A, Processing Mishap and Close Call Reports, for definitions of mishap types and close calls.) This directive applies to all elements of GSFC, to its contractors to the extent specified in their contracts, and to other organizations as specified in written operating agreements. Mishaps or close call investigations involving remote deployments at or by Wallops are managed by the Wallops Safety Office. This directive does not apply to investigations concerning civil, criminal, or administrative culpability or legal liability. The safety investigative process shall not be used to direct or justify disciplinary action.
