This procedure applies to:

a) all Network Rail staff involved with failures of infrastructure equipment;

b) all reported failures or irregularities of the following equipment for which Network Rail is responsible occurring on Network Rail Controlled Infrastructure:

i) signal;

ii) Automatic Warning System (AWS);

iii) Train Protection Warning System (TPWS);

iv) European Rail Traffic Management System (ERTMS);

v) Automatic Train Protection;

vi) Transmission Voice-Machine (TVM).

PURPOSE

This procedure provides a risk based approach to manage incidents involving failures of:

a) signalling equipment;

NOTE: This includes both right side and wrong side failures (WSFs).

b) track defects; and

c) WSFs of lineside safety equipment involving:

i) Track Circuit Operating Device (TCOD);

ii) Lookout Operating Warning System (LOWS); and

iii) Automatic Track Warning System (ATWS).

It sets out the requirements to be agreed between Network Rail and Train Operating Companies (TOCs) and Freight Operating Companies (FOCs) in relation to the receipt and response to form RT3185 for faults for which Network Rail is responsible (as defined in RIS-8047-TOM).
