This Guidance Document applies to LU and its suppliers operations related to LU business. It outlines reporting and investigation of incidents and the arrangements for communicating regulatory or other urgent concerns to affected parties. Details of the requirements to be adopted within LU are outlined in the Category 1 standard 1-556 and Category 5 standard 5-557.

Purpose

The purpose of this Guidance Document is to guide users to meet the requirements of Category 5 standard 5-557 and ensure that all incidents (including near misses) are recorded on an eIRF at the very earliest opportunity, usually by the person responsible for the location / activity giving rise to the incident or the person affected.
