This Guidance Document applies to all incidents arising from LU capital and maintenance works undertaken by LU or its supply chain.
Purpose
This document provides guidance on the use of the LU Electronic Incident Reporting Form, as required by Standards 1-556 and 5-557, to ensure that all incidents (including near misses) are recorded on an eIRF and tracked to closure.
