This standard is directed at staff attending any irregular operation, wrong side failure incident or accident involving, or allegedly involving, signalling equipment and those having any involvement in the restoration or repair of signalling and control systems.

Note: This standard supplements the requirements of LU Rule Book 2 'Managing incidents' which should be read in the first instance.

Purpose

This standard defines the minimum requirements for incident management, applicable to all failures or disturbances of London Underground (LU) signalling and control systems, however they are reported. This standard also details the requirements for the preservation of evidence for further examination.
