The document applies to Safety-Related Events associated with an Earthwork or a Structure: it defines the system for rating such Events, and the management of reports and registers of them. Most Events of interest concern the actual, or potential, derailment of a train or collision of a train with an obstruction. This document defines the means of rating such Incidents, and also of Failures that led to, or might have led to, them. Other Incidents and Failures should be rated by analogy with those covered or be based on engineering judgement.
This Work Instruction is particularly relevant to;
• Director of Civil Engineering, Analysis Engineer (Civils), and Civil Engineer (Geotechnical)
• Territory Civil Engineers, Territory Structures Engineers, Territory Earthworks and Drainage Engineers, and Territory Assurance Engineers (Civils)
Purpose
As part of the management system for ensuring the safe operation of the rail network, it is essential that Safety-Related Events are investigated, reported and followed up as necessary. The purpose of this document is to define the processes for rating and reporting Safety-Related Events1 associated with an Earthwork or a Structure.
In pursuit of uniformity within the Company, the system used to rate Safety-Related Events associated with Earthworks and Structures, and the process for managing reports of them, are similar to those given in RT/E/S/10047: Management of Safety Related Reports for Signalling Failures and RT/E/S/30047: Management of Safety Related Reports for Telecoms Failures.
