PURPOSE. In accordance with NPD 8621.1, "NASA Mishap Reporting and Investigating Policy," NASA policy is to report, investigate, and document NASA mishaps and determine the root cause(s) of NASA mishaps in order to develop and implement corrective actions. The purpose of this Directive is to explain the processes involved in close call and mishap reporting, investigation, and recordkeeping; explain how the program will be conducted in compliance with Occupational Safety and Health Administration (OSHA), NASA, and Marshall Space Flight Center (MSFC) requirements and policies; and establish responsibilities and guidelines for employees to prevent injury, illness, and loss or damage to NASA/MSFC property.
APPLICABILITY. This Directive is applicable to all persons on MSFC property and contractors involved in MSFC operations.
