PURPOSE
To identify the Center's instructions and processes for reporting, investigating, and documenting mishaps and close calls as required by NPR 8621.1.
APPLICABILITY
This MWI applies to Center personnel, programs, projects, and activities, including contractors and resident agencies to the extent specified in their respective contracts or agreements. ("Contractors," for purposes of this paragraph, include contractors, grantees, Cooperative Agreement recipients, Space Act Agreement partners, or other agreement parties.)
The MWI applies to the Michoud Assembly Facility (MAF).
This MWI applies as follows: all mandatory actions (i.e., requirements) are denoted by statements containing the term "shall." The terms: "may" or "can" denote discretionary privilege or permission; "should" denotes a good practice and is recommended, but not required; "will" denotes expected outcome; and "are/is" denotes descriptive material.
This MWI applies the following: all document citations are assumed to be the latest version unless otherwise noted.
This MWI applies to the reporting of all Center mishaps and close calls. 2.6 This MWI applies to investigating and documenting "only" Center mishaps and close calls that have been identified as Type C, Type D, and Center Close Calls.
This MWI does not apply to investigating and documenting Center mishaps and close calls that have been identified as Type A, Type B, and High-Visibility Mishaps or Close Calls, or mishaps that involve NASA aircraft. These types of mishaps and close calls are investigated and documented following the processes identified in NPR 8621.1.
